Step #1 - Enrollment Form/Cholesterol Screening
Fill out an enrollment form.
This form is designed to help assess your risk for heart disease
and is used for screening purposes. About 5 minutes are needed to
complete it.
A one-time (with continued renewal), non-refundable enrollment fee of $35 is required when
you return your enrollment form. This fee is used to process your
membership information and for the cholesterol screening at Athens
Medical Lab. We must have your enrollment form and cholesterol results
before you can move forward in the enrollment process. Enrollment
forms kept on file for three months only.
Step #2 - Health and Fitness Evaluation
Within 24 hours of receiving your cholesterol results (and we have your enrollment form), you will receive a phone call to schedule your health and
fitness evaluation. Those who are at low risk with no contraindications
to exercise will schedule an appointment for an assessment. These
assessments are scheduled on Mondays/Wednesdays 10:00 a.m.
- 12:00 noon and Tuesdays/Thursdays 4:00 - 7:00 p.m. Each appointment
will last between 45-60 minutes and will include a series of resting
and physical assessments including:
Resting heart rate, blood pressure, height,
weight, waist circumference, pulmonary function testing, body composition
with skin fold calipers (optional), a YMCA bike assessment for cardiorespiratory
fitness, push-ups and sit-ups for muscular endurance, and sit and
reach assess for flexibility.
Those who have a metabolic disease may
be asked to schedule a stress test. (Note: An additional fee may
accompany the stress test.)
Step #4 - Orientation to WellWorks
During your orientation you will be introduced to the front desk,
fitness center and locker rooms. A Fitness Center Supervisor will
work one-on-one with you to show you how to adjust and use the fitness
center equipment properly and safely develop a stretching routine.
The orientation will last approximately 30 minutes, and may be scheduled
at any time during regular business hours.
With this information in hand, you now have the tools to start making
heart healthy choices, leading to and maintaining a healthy lifestyle.
