This form is intended to give us some feedback from your employer/sponsor about how you did on the job.

If you are taking the internship for a grade -- and only if you are taking it for a grade -- please make sure that your sponsor fills out and returns the performance evaluation form.

It is your responsibility to print the evaluation form, give it to your internship sponsor, and make sure they send the form back to the Director.
* The form should be sent during the last two weeks of your internship.
* Please keep in mind that we cannot give you a grade until all of the paperwork is done, including the sponsor's evaluation. If you are running into problems, contact the Director as soon as possible.
* The performance evaluation should be sent to the following address:
