| Ohio University Transmittal Form for Proposal Review and Approval |
| According to Ohio University policy "Guidelines for
Sponsored Research" (19.051), all proposals for external funding must be reviewed and approved by the
Vice President for Research prior to submission to an external agency. To obtain this approval, a copy of the proposal, along with any forms requiring institutional signature,
and the completed and signed copies of the "Ohio University Transmittal Form
for Proposal Review and Approval" must be submitted to the Office of Research and Sponsored Programs.
Please complete the current version of the three page form, obtain appropriate signatures [the Project Director; Project Investigators/ Co-Investigators; Center or Institute Director, if appropriate; Department Chair/ School Director(s); and Dean (s)], and bring the form along with your proposal to the Office of Research and Sponsored Programs to obtain the required institutional approval and signature. This procedure is required for all applications submitted by Ohio University faculty and staff, even if the funding agency does not require a university signature. Projects that are continuing do not require a transmittal form, however the "Space Utilization Form and Conflict of Interest Certification" must be updated annually.
